Physician Visits Benefit: A FLAC will pay based on the
benefit level chosen:
Level 1: $15 (3 visits pcy for an individual and 6 for a family) Hospital Confinement Benefit: AFLAC will pay the amount per day for the level chosen when a covered person required confinement for 14 or more hours for a covered sickness. Must be related to a sickness or for rehabilitory care as a result of a sickness: Level 1: $50 (days 1-15) - $100 (days 16-180) Initial Hospitalization Benefit: $250 per period of confinement when a covered person is confined to a hospital for 24 hours. Once per calendar year per covered person. Major Diagnostic Exams: $150 for the following exams: CT Scan, MRI, EEG, Thallium stress test, Myelogram, Angiogram, Arteriograms Surgical Benefit: $100-$2,000 when a covered person has surgery performed in a hospital or ambulatory surgical center based upon the Schedule of Operations in the policy. Only one benefit per 24 hour period for the surgery even though more then one surgery may be performed. Ambulance Benefit: $100 for ground transportation and
$1,000 for air ambulance. Limited to two trips per calendar year.
