than the usual 12 month) for crowns. It is a PPO plan, so you are free to go outside the network with
any dentist of
your choice. And, although there is a $1000 cap on benefits, Diagnostic and
Preventative Services (routine cleanings and x-rays) do not count towards this
annual cap.

services nor do not count towards your
maximum annual allowance.
United Concordia Dental
Individual PPO Dental Coverage Plan
(NOTE: After using the

This dental plan is fully insured through United
Concordia. There is a $1,000 yearly maximum per person
(but only on major services) with a $50/$150 deductible per person/ per family. It
includes the Preventive Incentive Program that provides 100%
coverage In-Network for Diagnostic/Preventive Services
(These charges are excluded from the $1,000 yearly maximum).
There is NO WAITING PERIOD on routine cleanings, x-rays, fillings, extractions,
oral surgery and root canals. The Waiting Period for Class III
Major Services (crowns) that are not covered however, is 6 months
rather then then the normal 12 months you will find on other PPO plans. Also, the out-of-network benefits are the same
as in-network. This PPO plan uses the
United Concordia Advantage Plus
network (remember to select the "Concordia Advantage
Plus" when asked what network you want...), meaning that if you choose a network provider then
they accept the negotiated network rate and everything is
covered 100%. If you go outside the network then the plan
pays usual and customary based on the negotiated network
rate and you pay the difference if the out-of-network
dentist charges more.

Delta Dental's PPO Plans - DeltaCare® USA
Delta Dental
Plans:

Delta Dental offers 6
different plans. There are two networks, the PPO
network and the Premier network, and under each of these
networks, you have the option of selecting either the
Gold plan, the Platinum plan or the Diamond Plans. This makes up the
6 policies offered by Delta Dental.

If your dentist is not in
network with any of the plans, it is best to go with one
of the Premier plans (either gold, platinumor diamond) because
they pay out based on a UCR (usual, customary, and
reasonable charge) rather than the reduced PPO fee
schedule you would encounter in the PPO network plans. The pay-out amount for the different dental
procedures tend to have a higher dollar amount on the UCR than they do with the PPO fee schedule, so by
selecting a Premier plan when going out of network you
will have a lower out of pocket expense.

If your dentist is not in
network for the plan, they can balance bill for the
difference between what they charge for the dental work
and what Delta allows for that same work (this is why it
is best to go with Premier when going out of network).
It is always best to select
a plan that your dentist is in network for.

The network of dentists
with the Premier plan is much larger than the network of
dentist with the PPO plan.

dentist from the DeltaCare USA network from whom you will
receive most treatment. When
